1. Check in when you arrive for your appointment.

2. You will fill out a questionnaire if you are a new patient. Fill out any applicable state forms.

3. After the forms are complete, return them to the front desk and we will process your paperwork.

4. We must copy your state identification card for our file and to send with your application.

5. You will then meet with our doctor. He will do a brief history and physical exam. He will then fill out the necessary state forms if you qualify.

6. We will make copies of all forms and medical records for your file at our office. You will also have your I.D. returned to you at this time. You pay our clinic’s fee here.

7. You will receive the originals and copies of your state medical marijuana application forms and any medical records you brought in with you.
